Significantly more intense pain in methadone-maintained patients who are addicted to nicotine

چکیده
Pain and cigarette smoking are very common in heroin-dependent patients (HDPs) receiving methadone maintenance treatment (MMT) and both have substantial negative effects on HDPs' physical and mental health. Nevertheless, very few studies have assessed the relationship between the two in HDPs. This study examined the association between pain intensity and smoking in Chinese methadone-maintained HDPs. A total of 603 HDPs were consecutively recruited from three MMT clinics in Wuhan, China, and administered with a socio-demographic and drug use questionnaire, a smoking questionnaire concerning average number of cigarettes smoked daily and Heaviness of Smoking Index, and Zung's Self-rating Depression Scale. We used a five-point Verbal Rating Scale to rate the intensity of pain. To determine whether pain's associations with smoking and nicotine dependence were independent, an analysis of covariance was adopted to control for the potential confounding effects of socio-demographic variables, drug use characteristics, and depressive symptoms. Net of potential confounders, in methadone-maintained HDPs, pain intensity was significantly higher in smoker than non-smokers (F = 11.836, P = 0.002) but the pain intensity did not differ significantly across patients with various levels of cigarette consumption (F = 1.992, P = 0.137), while the pain intensity significantly differed across patients with different levels of nicotine dependence (F = 3.252, P = 0.013). Pain is significantly associated with smoking in HDPs receiving MMT, this phenomenon may be explained by the association between nicotine dependence and pain.

Pain of methadone-maintained heroin addicts: lonelier individuals feel more intense pain
Managing pain in patients with heroin addiction is challenging, because most pain medications are addictive. A promising way to relieve pain of heroin addicts is to identify and address modifiable psychosocial factors associated with pain. This study examined the association between loneliness and pain intensity in Chinese heroin addicts receiving methadone maintenance treatment (MMT). A consec...
متن کاملAuditory target processing in methadone substituted opiate addicts: The effect of nicotine in controls
BACKGROUND The P300 component of the auditory evoked potential is an indicator of attention dependent target processing. Only a few studies have assessed cognitive function in substituted opiate addicts by means of evoked potential recordings. In addition, P300 data suggest that chronic nicotine use reduces P300 amplitudes. While nicotine and opiate effects combine in addicted subjects, here we...

Background: Addiction is a polygenic disorder caused by genetic and environmental factors. The opioid material can act as an epigenetic element, like DNA methylation. The present study aimed to examine the effect of epigenetic drugs such as nicotine, morphine, methadone, and buprenorphine on the methylation of two CpG sites in promoter of Oprm1 gene in male Wistar rats. Materials & Methods: In...

BACKGROUND AND OBJECTIVES The use of opioids for cancer pain has already well established by several well-controlled clinical trials. However, there is a major controversy about long-term use of opioids in non-malignant chronic pain, which has been significantly intensified in the last few years. This study aimed at evaluating available data on the use of opioids for treating non-malignant chro...
متن کاملOutpatient Detoxification Completion and One-Month Outcomes for Opioid Dependence: A Preliminary Study of a Neuropsychoanalytic Treatment in Pain Patients and Addicted Patients
The current practice in the United States is to maintain patients with opioid dependence on opioids. A case series approach was used to investigate the results of outpatient detoxification/opioid-free treatment using a neuropsychoanalytic paradigm. Detoxification involved a single dose of buprenorphine, adjunctive medications, and intensive neuropsychoanalytic psychotherapy. Depression, attenti...
